The prevalence of overweight/obesity has increased significantly over the past four decades, affecting more than half of adults in China. Overweight/obesity is often accompanied by various diseases such as hypertension, diabetes, dyslipidemia, hyperuricemia, etc. Multiple studies have shown that overweight/obese patients can significantly improve metabolic disorders and reduce the risk of obesity by 2% if they lose 5% to 15% of their body weight. Mazdutide is a dual receptor agonist of glucagon-like peptide-1 receptor (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP), used to treat obesity and type 2 diabetes. Recently, Nature Communications published a Phase 2 trial evaluating the use of mazdutide in overweight/obese adult patients in China, which showed that treatment with mazdutide 6 mg for 24 weeks can significantly reduce body weight and is safe and effective.
This is a randomized, double-blind, placebo-controlled phase 2 trial that included 248 Chinese patients with overweight [body mass index (BMI) ≥ 24 kg/m2] with overeating and/or at least one condition from June to October 2021. Adult subjects with obesity-related comorbidities, or obesity (BMI ≥ 28 kg/m2), all subjects were randomized to receive mazdutide 3 mg once a week in a ratio of 3:1:3:1:3:1 (62 patients ), 4.5 mg (63 patients), 6 mg (61 patients), or matching placebo (62 patients). The average baseline weight of all subjects was 89.4 kg, and the average baseline BMI was 31.8 kg/m2.
The primary endpoint was the percent change in body weight from baseline to 24 weeks of treatment. The results of the study showed that from baseline to week 24 of treatment, the body weight percentages of the mazdutide 3 mg, 4.5 mg, 6 mg and placebo groups decreased by 6.7%, 10.4%, 11.3% and increased by 1.0%, respectively. Compared with the placebo group, the estimated treatment differences of mazdutide 3 mg, 4.5 mg, and 6 mg were -7.7%, -11.4%, and -12.3%, respectively (P<0.0001). At week 24, the weight loss of subjects in all doses of mazdutide group was greater than that of the placebo group (P<0.0001).

Compared with the placebo group, the proportion of subjects in the mazdutide group at all doses who lost 5% or more or 10% or more of body weight from baseline was significantly higher (P<0.0001). At week 24, the proportions of subjects in the mazdutide 3 mg, 4.5 mg, 6 mg and placebo groups who had lost 5% or more of their body weight compared with the baseline were 58.1%, 82.5%, 80.3% and 4.8% respectively; the weight loss was 10% compared with the baseline. % and above were 19.4%, 49.2%, 50.8% and 0% respectively.

In summary, compared to placebo, treatment with Mazdutide 3 mg, 4.5 mg, and 6 mg for 24 weeks in Chinese overweight/obese adult patients can achieve clinically significant weight loss, and doses up to 6 mg also demonstrate good tolerability and safety. The research results support Mazdutide as one of the long-term weight management strategies.
